What Is An Annular Tear?

An annular tear is a split or crack in the tough outer ring of a spinal disc. The outer ring, called the annulus fibrosus, surrounds the disc’s softer center. Tears may develop from strain, repeated loading, or age-related changes. An annular tear may cause local back or neck pain and may coexist with disc bulging.

Key Takeaways About Annular Tear

  • The injury affects a disc’s outer annulus fibrosus.
  • Pain may occur near the affected spinal level.
  • An annular tear is not the same as a herniated disc.

How Annular Tear Works

A medical model of a spinal disc sits beside a card reading “ANNULAR TEAR” on a desk with a stethoscope and pen

An annular tear affects the layered outer ring of a spinal disc. The annulus fibrosus helps contain the softer inner disc material and supports movement between nearby vertebrae. A tear may irritate nearby tissues, especially when bending, lifting, or maintaining one position places stress on the affected area.

Healthcare professionals assess an annular tear through the person’s symptoms, physical examination, and imaging when appropriate. Imaging may show changes in the disc, while an examination checks movement, strength, sensation, and related nerve findings. An annular tear may remain limited to the outer ring, or disc material may extend through the damaged area and form a disc herniation. Disc degeneration may also occur alongside an annular tear.

From the Arrowhead Clinic - Lithia Springs team

When we discuss an annular tear, we connect the imaging description with the person’s symptoms, movement limits, and examination findings. Our team considers how the disc finding relates to the requested herniated disc treatment and avoids treating an image as the complete clinical picture.

Annular Tear in Practice: A Real-World Example

A person seeking herniated disc treatment may report lower back pain after repeated lifting, while imaging describes an annular tear near the lumbar spine. The examination helps determine whether the symptoms appear local or involve related nerve irritation.
